保密协议

Our staff cannot reveal nor confirm the identity of students seeking counseling services, 他们参与这些服务, nor the content of communications with those students to those outside the 咨询办公室 Services staff without the student’s knowledge and written consent. Information about a student’s use of counseling services is kept separately from their academic record. Students may sign a written release granting their counselor permission to share confidential counseling information with specific other persons such as parents, RA, 教员, 或者其他医疗服务提供者.

咨询服务 is committed to maintaining the confidentiality of your counseling appointments. It is the philosophy of 哥伦比亚大学 咨询服务 that one of the basic rights of clients in treatment is the right to confidentiality. 保密 is an ethic that prevents unauthorized disclosures about clients, 包括他们的名字和照顾, 未经客户明确书面许可, 除非有虐待或忽视儿童的嫌疑, 虐待或忽视年老或残疾的成年人, 怀疑对自己或他人有严重威胁, 或者通过法院命令.

All student client files are clearly marked “Confidential” and housed in the 谘询服务主任’ locked office, 在一个锁着的壁橱里, 文件或书桌. 办公室的钥匙不在通用锁系统上.

The Director’s Administrative Assistant and all student workers in the office are trained in the proper handling of confidential issues such as identity, 访问, 电话, 等. All sign a “保密 Statement” agreeing to comply with principals of confidentiality.

辅导员以外, only the Administrative Assistant has access to student client data (not progress notes), and for the sole purpose of data gathering at the request of the Director.

E-mail is not considered a secure form of communication; therefore, 在电子邮件中不能保证机密性. Please be aware that e-mail accounts may only be checked infrequently and not at all on weekends or holidays. Any clinical information sent over 电子邮件 will be discussed with the student in person or over the phone; that information becomes part of the clinical record.

If a student client requests that confidential information be released to/obtained from a third party, an “Authorization to Release or Obtain Confidential Information” form. This form includes an explanation of the requirements of the South Carolina Code that protects confidential mental health information. 通过联系辅导员, the student client may revoke her permission to release confidential mental health information at any time.

When 咨询服务 receives a request for a student’s confidential mental health information, 该学生将收到有关请求的通知.

在摄入, a student client is provided information that states the student’s rights with respect to uses and releases of confidential mental healthcare information. The student client is also provided with this “Protection of 保密” information.

咨询服务 will provide any person with a copy of this “Protection of 保密” information upon request.

咨询服务 will retain a copy and any revisions of the “Protection of 保密” for at least five (5) years.

The Counselor may contact you to follow up on select referrals from family, 教师, 员工还是朋友, to inform you of other services that may be of interest to you or to respond to your call, 电子邮件, 等.

The “Protection of 保密” information is posted on 咨询服务’ webpage for access to all individuals who may seek counseling.

如果你认为你的隐私权被侵犯了, 你有权表达你的担忧. You may address your concerns to the Director of 咨询 in writing or to the Dean of Students.

业务范围

Students whose needs cannot be accommodated within short-term counseling, or whose needs require a particular type of expertise unavailable on campus, 是否参考社区资源.   Services provided on campus will NOT be an appropriate substitute for intensive, 或者长期治疗.   咨询服务 will provide referral services when it becomes clear that the student’s needs are outside of the scope of care. The counseling services team will follow-up with the student to facilitate the referral, 提供必要的协助.
